Prevention of Sequelae in Postoperative Coarctation

Abstract
Since surgical repair of aortic coarctation became available in 19451), survival of patients with aortic coarctation has dramatically improved and the number of patients who were operated on and reach adulthood is steadily increasing. Surgical options for repair of coarctation include excision with end to end anastomosis, excision with extended end to end anastomosis, subclavian flap repair, patch aortoplasty, interposition of grafts, and various modifications. The possible complications both during and following repair is extensive and their prevention requires careful attention to all details of the techniques. The early complications include paraplegia, hemorrhage, paradoxical hypertension, chylothorax, left recurrent laryngeal nerve palsy, phrenic nerve palsy, Horner¡®s syndrome, left arm ischemia. Late complications include hypertension, recurrent coarctation, aneurysm formation, cardiovascular accidents, premature coronary atherosclerosis, and exercise- induced hypertension.
